Position: Remote Patient Monitoring Nurse

Telenett is a leading provider of telehealth services, dedicated to improving the health and well-being of patients through remote patient monitoring. We are seeking a highly skilled and compassionate Remote Patient Monitoring Nurse to join our team on a full-time basis.

As a Remote Patient Monitoring Nurse, you will play a crucial role in providing quality care to patients in the comfort of their own homes. You will use advanced technology and telecommunication tools to monitor and assess patients' health status, provide education and support, and collaborate with other healthcare professionals to develop individualized care plans.

Key Responsibilities:

- Utilize telehealth technology to remotely monitor patients' vital signs, symptoms, and medication adherence

- Conduct virtual assessments and communicate with patients to identify any changes in their condition

- Provide patient education on disease management, medication management, and lifestyle modifications

- Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans

- Document patient interactions and progress accurately and in a timely manner

- Maintain confidentiality and adhere to HIPAA regulations at all times

- Participate in ongoing training and education to stay updated on the latest telehealth practices and technology

- Communicate effectively with patients and their families, providing emotional support and addressing any concerns or questions they may have

Qualifications:

- Valid RN license in the state of practice

- Minimum of 2 years of nursing experience, preferably in a home health or telehealth setting

- Experience with remote patient monitoring technology and telecommunication tools

- Strong critical thinking and assessment skills

- Excellent communication and interpersonal skills

- Ability to work independently and in a team environment

- Knowledge of HIPAA regulations and patient confidentiality

- BSN degree preferred but not required
