Telenett is seeking a dedicated and compassionate Case Management Nurse to join our team in Albany, GA. As a Case Management Nurse, you will be responsible for coordinating and managing the care of patients with complex medical needs. Responsibilities:
- Collaborate with healthcare team to develop and implement individualized care plans for patients with complex medical needs
- Monitor patient progress and make necessary adjustments to care plans
- Educate patients and their families on treatment plans and self-care techniques
- Coordinate with insurance companies and other healthcare providers to ensure proper coverage and continuity of care
- Conduct regular assessments of patient needs and provide appropriate interventions
- Advocate for patients and ensure they receive quality and timely care
- Document all patient interactions and maintain accurate records
- Stay up-to-date on medical advancements and best practices in case management
- Participate in team meetings and case conferences to discuss patient care and progress
- Adhere to all HIPAA and patient confidentiality guidelines
Requirements:
- Valid RN license in the state of Georgia
- Minimum of 2 years of experience in case management or a related field
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Excellent organizational and time-management skills
- Knowledge of medical terminology and procedures
- Familiarity with insurance processes and regulations
- Compassionate and empathetic attitude towards patients
- Ability to handle sensitive and confidential information with professionalism and discretion
